Transcript and Immunization Record Request and Release Procedure
The Letchworth Central School District is committed to protecting student privacy while ensuring timely access to education records. Student transcripts and immunization records are part of a student’s education record and are protected by the Family Educational Rights and Privacy Act (FERPA) and New York State Education Law §2‑d.
Who May Request a Transcript or Immunization Record
The following individuals may request a student transcript and/or immunization record:

Students who are 18 years of age or older or who have graduated

Parents or guardians of students under age 18

Parents of students who are 18 or older, only with written consent from the student (unless the student is a verified IRS dependent)

Another school or school district in which the student intends to enroll
Colleges, employers, scholarship organizations, and other third parties may not receive transcripts without written student consent.
How to Request a Transcript and/or Immunization Record
Preferred Method: Written Request
The safest and fastest way to request a transcript is through a written request on the District’s Records Release request form. Request forms are available upon request by contacting the District Registrar by phone or email. Incomplete forms will not be processed. Completed forms should be submitted to the District Registrar in person, by mail, or via email at the address below.
Phone Requests
Phone calls may be used to ask questions or begin a request, but the transcript and/or immunization record will not be released based solely on a phone call. This protects students and families from unauthorized access to confidential records.
